1) Assessing Your Practice – provided by the DATA team, Abt Associates. This is a large pdf file, but we thought that in particular pages 2-4, 7-8, 13-16, 19-21, 24-28, 34 may be helpful to your team.
2) PCHHC Pilot Practice Survey – this was piloted by Dr. Frayne when he was working with the Magnolia Clinic in Florida. This pre-intervention practice survey gathers information about the practice itself, but also about some current preconception risk screening activities.
3) Cross-Functional Flowchart & Matrix Templates – These have been used extensively in primary care and OB settings to document patient flow and roles & responsibilities of staff.
