Improving health and wellness requires a multi-faceted approach! This section provides information about the work we currently have underway.
Our newest initiatives both focus on improving the health and well-being of women who are of reproductive age and live with chronic conditions. This includes women who experience chronic conditions warning signs during pregnancy (such as gestational diabetes) and women who leave pregnancy with newly acquired / diagnosed conditions. There is an urgent need to build women-centered systems of care to support women in living their healthiest lives before, between and beyond pregnancy. Our PCORI Engagement Project is a one year research project that provides the opportunity for us to be in community with 40 African American and Native American women and listen with intention to their strategies for change. Our Connecting the Dots II: Improving care for Women with Chronic Conditions Project provides us with the opportunity to bridge across a variety of care silos and create a Collaborative of Change Agents who can help fashion a Road Map for Change.
We are also excited to be continuing our Preconception Collaborative Improvement and Innovation Network Project to design a new set of patient educational materials and share our learning and strategies for improving preventive care for women. We have a wealth of resources and materials available to anyone interested in improving wellness care!
Our Show Your Love campaign focuses on providing person-centered, encouraging and actionable information to people of child bearing age about health and wellness. We are continuing to update the content of the Policy & Finance and Surveillance & Data pages – stay tuned!
